Prior to your scheduled planning session, you will need to complete an intake form and Planning Session Agreement. You will need to bring your completed intake form with you to your Planning Session. If your Planning Session is via phone or video conference, you will need to scan and email your intake form to our attorney at prior to your scheduled Planning Sesssion.
